OVERVIEW OF SERVICES

EMPLOYMENT SUPPORTS:

This program will focus on support while the person served gradually does increasing percentages of the actual work. The goal of the program is employment, based on the needs and choices of the individual and the resources of the community.

PREPARATION:

Assistance to develop appropriate career goals and to work towards achieving those goals. WIN shall provide 200 units of individual service to five (5) individuals as identified in the Entrance Criteria in Section 4

PLACEMENT:

Support in the actual finding and maintaining of employment. WIN shall provide 1450 units of shared and individual service to fifteen (15) individuals as identified in the Entrance Criteria in Section 4 .

COMMUNITY ACCESS:

This program will focus on providing opportunities to participate in valued activities within the community. The goal of this program is to optimize integration, independence, quality of life with acceptance and full participation in the community. To enhance the acceptance of people with disabilities by providing support to prepare for, coordinate and take part in community activities. WIN shall provide 3700 units of shared and individual service for up to twenty four (24) individuals as identified in the Entrance Criteria in Section 4.

COMMUNITY LIVING SUPPORTS:

To support individuals with developmental disabilities in a spectrum of residential support and/or accommodations through the use of generic community resources. The goal of this program is to offer opportunities for growth and development within the least restrictive environment, while
accommodating personal safety concerns.

OVERNIGHT STAFFED RESIDENCES:

Up to 24-hours a day support in a shared living situation. Support will be provided based on individual needs and circumstances. WIN operates one (1) home which is funded by Individual
Funding Agreements and this contract.

This Overnight Staffed Residence is located at 9723 - 99 Street, Westlock, Alberta T7P 1Y5.
WIN shall provide 1095 units of shared and individual service to three (3) individuals as identified in the Entrance Criteria in Section 4.

SUPPORTED INDEPENDENT LIVING:

Support in the family and/or person’s home, aimed at the building of independence and overall development of the person served. These persons may have a higher level of independence or another network of supports where WIN staff are not as heavily involved.

ADMINISTRATION:

This program provides support to persons served and their families in a variety of ways:

Information Source
  • Keeping up-to-date on direction of local, regional and provincial organizations i.e. North East Consortium of Service providers, AARC, Inter-Agency etc.
  • Maintaining and/or obtaining information as required, i.e. available funding, services, equipment or support.
Personnel Support
  • Assistance in all areas of staffing i.e. hiring, screening, training, monitoring etc.
  • Providing short term casual staff
  • Complete payroll and cheque processing
  • Completing necessary reports, i.e. Receiver General, Revenue Canada, WCB etc.

Financial and Facility Support
  • Conduct fund raising campaigns to address specific needs
  • Offer use of WIN’s office facilities to related community groups.

Consultation
  • Supporting persons served and their families to access generic services,
  • Advising/Lobbying community groups/services on the needs of people with disabilities

Short term Support
  • to assist individuals who don’t have individualized services in place, or in areas not addressed by their individual service plans.
  • The goal of this program is to address the current concerns and develop plans to ensure that these concerns are addressed.

Eligibility Criteria and Regional Intake Process

  • Only Individuals with a developmental disability who meet PDD eligibility criteria and have been deemed eligible through the PDD intake process are to be provided with PDD funded services
  • The applicant and/or referring party will be referred to the PDD Service Coordinator to determine eligibility prior to being accepted for service by the service provider. To be eligible for PDD supports a person must meet the following criteria:
    • be 18 years of age or greater
    • be living in Alberta
    • have a disability, which is assessed as a “developmental disability”, using PDD criteria.
  • Individuals who do not meet these criteria may be referred to other services. In the event that an individual is denied service through PDD, the individual can follow the PDD appeal process and shall be informed of the process by the PDD Service Coordinator.
  • Once deemed eligible for PDD supports, all supports provided will be based on an individually negotiated service plan.

Service Provider Entrance and Exit Process

REFERRAL, ENTRANCE AND EXIT
Entrance
  • Applicants must be diagnosed as having a developmental disability.
  • Applicants must be between the age of 18 and 65 years old.
  • Agency must be able to provide identified program service to meet the needs of the individual.
  • Applicants must be able/willing to participate in the program they help design to meet their needs.
  • Applicants who display maladaptive behaviours (aggressive, destructive or illegal) should be able to respond to intensive behaviour modification techniques implemented by agency staff in consultation with behavior specialists.
  • Applicants must show the potential to cooperate/participate in agency routine and procedures in a training environment.
  • Applicants medical needs must be such that trained lay staff or community medical resources can adequately
  • Provide the day to day level of care required.
  • Priority will be given according to the need of the person served/family and the ability of agency to meet this need.
  • The person served will be responsible for the cost of their own transportation
Exit
  • Planning Team will decide when person served no longer requires services.
  • Readiness for advancement will be determined based on: willingness of the person served (and/or family ) to accept more responsibility and independence, priorities set down by the Planning Team,
  • Person served/guardian express a desire to voluntarily withdraw from services.
  • Person served encounters a medical problem that exceeds the capability of the agency or community resources on a long term basis. These consumers will be considered for re-admission in the event these problems become manageable as per level of services available.
  • Services are discontinued by WIN because of a significant lack of cooperation on the part of the person served and/or parent/guardian or other reasonable cause. This decision must be supported by documentation.
  • Suspension or dismissal from one program within the continuum would not necessarily constitute grounds for a consumer to receive a similar consequence in other areas.
  • The Individual Services Coordinator, Persons with Developmental Disabilities Board, will be involved in the Planning throughout.


Service Provider Appeal Procedures

APPEAL/GRIEVANCES PROCEDURE:

It is the position of WIN that good communication is important and will be done best by direct contact which involves only the individuals with a conflict in the initial stages of the conflict, i.e., between consumer/guardian and their immediate support worker.

WIN encourages all concerned to bring forward concerns that they feel are valid. All concerns will be dealt with as soon as is possible in a fair and impartial way.

Filing a Grievance is a serious matter and will not normally be considered without the individual/parent/guardian having first followed the normal steps of communication and protocol.

If all reasonable efforts to resolve a conflict situation in an informal manner between parties directly involved are not successful than the individual/parent/guardian may file a grievance using the format provided by WIN. This format will comply with C.E.T. Standards.
